Clinical Case Manager
M – F 9AM – 5PM with some evening and weekend hours. 100% in person.
The Clinical Case Manager I (CCM) is responsible for providing clinical and case management services to residents living in permanent supportive housing for seniors and disabled individuals formerly experiencing homelessness. The CCM follows the principles of Harm Reduction and Trauma-Informed care to engage a caseload of about 15 residents who are adults, families, and children with histories of homelessness and mental health diagnoses. CCMs are part of the agency’s Resident Services team and work in partnership with the Housing Operations department staff.
QUALIFICATIONS:
ยง ASW/ AMFT/ APCC
ยง Demonstrated ability in providing effective clinical services within a context of harm-reduction practices and voluntary services, and in-depth knowledge of harm reduction practices.
ยง Strong clinical observational and assessment skills.
ยง Demonstrated ability in providing services to individuals, especially those struggling with substance use and mental health issues or who have experienced homelessness.
ยง Experience in engaging residents, crisis intervention, conflict resolution, and meeting facilitation.
ยง Experience in creating and conducting trainings.
ยง Knowledge of supportive housing, housing first, trauma-informed, motivational interviewing and harm reduction principles,
ยง Proven ability to navigate San Francisco’s behavioral health care system and experience in advocating for access to services, including collaboration with other agencies.
ยง Excellent computer skills (Outlook, MS Word, Excel, Salesforce or other cloud based database).
