Job Information

The Social Worker helps post-acute patients and their families to navigate the skilled nursing rehabilitation experience. Coordinates with interdisciplinary team including nurses, therapists, and physicians regarding discharge planning. Serves as liaison with managed care partners and communicates with patients about insurance resources and limits. Assesses and advocates for patients’ psychosocial well-being through care planning and coordination of services. Ensures regulatory compliance and proper documentation in electronic health record. The Social Worker will also assist providers and Care Managers.

This is temporary, full time, 4 – 5 days/ week, M – F basic business hours. Temporary for 1 – 3 months.


  • Preferred knowledge of PDPM
  • Good with discharge process and coordination
  • Knowledge of Medicare
  • Must have BA, MA or MSW
  • Must have 3 years relevant experience in medical SW or with seniors, SNF experience preferred

We are looking to hire a part or full time temporary Program Manager to oversee staff and a program that serves at risk youth.  The can be part or full time, 20 – 40 hours/ week.  Masters is preferred, BA required.


-4 years’ experience in similar a management role. Strong experience in administration, coordination, program development, quality assurance, and case management supervision. Demonstrate a track record in meeting program quality/productivity standards. Excellent management skills, utilizing best practices in achieving staff engagement and quality outputs.
-Ability to travel between and work from various LA County area locations. This position is based in Inglewood, with travel to other offices in South LA, Hawthorne and Pacoima, as well as partner sites in Compton, downtown LA, etc.
-Expertise in typical/atypical child development and trauma-informed practice, with working knowledge of Evidence Based Practices (EBP) for adolescent behavior challenges.
-Experience working with adolescents and high risk populations, including crisis management.
-Experience/commitment to Motivational Interviewing and client-centered strengths-based practices.

This critical leadership position manages and ensures overall program operations and quality of the program, which is leading innovation that is transforming youth justice in LA and the state, with a commitment to both youth development and servicing victims of youth crime, while impacting systemic change to end the school to prison pipeline.
