Social Service Staffing is hiring for a 4 month temp or temp to hire REMOTE position as a Care Coordinator.  Candidate must be registered with the BBS or licensed. ASW/ AMFT/ LCSW/ MFT.

  • Assesses and links members in need of behavioral health services.
  • Provides excellent customer services to callers regarding a variety of behavioral health requests and inquiries.
  • Documents clinically concise information regarding calls, services and provider needs.
  • Reviews clinical information on members including, but not limited to their presenting behavioral health symptoms, needs, past diagnosis, treatments, and past treatment history.
  • Uses a screening tool to help determine if client meets the mild to moderate services category or is more appropriate for Specialty Mental Health Services through Behavioral Health and Recovery Services.
  • Assists members in understanding their benefits and make decisions about their care and services using existing protocols and procedures.
  • Helps link members to a variety of services and treatment providers via referral and resources.
  • Consults with various service providers throughout the medical, behavioral health, and substance use communities to help clients find the appropriate referral.
  • Periodically reviews hospital records and other clinical records regarding specific cases to help inform decisions about their care and services using existing protocols and procedures.
  • Screen callers for risk and safety, suicidal / homicidal ideation, and links them to crises services when indicated based on existing protocols and procedures.
  • Works collaboratively with Call Center staff.
  • Reviews Assessment and Treatment Plans as needed by the team.
  • Responsible for the early identification and assessment of members for potential inclusion in a comprehensive case management program. Refer members for Case Management accordingly
  • Tracks provider availability in screening tool or system.
