This is temporary, full time, starts ASAP and will go through Jan. M – F basic business hours. Under supervision of the Supervisory Case Manager, this position will be responsible to provide case management, housing readiness assistance, housing searches and placement services and operational oversight to homeless individuals in order to facilitate their progress from homelessness to stability and eventually self-sufficiency.

The Program includes 17 trailers and a 4-bedroom house located at 3 sites. Clients at these sites will be eligible for placement in either PSH, NED, COACH or RRH, depending on eligibility. Respite staff will work with clients to get them housing-ready, conduct housing searches based on housing type, assistance with the housing application and move-in process.

Essential Duties and Responsibilities

  • Responsible for client intake process, initial assessment, and housing plan development.
  • Provide daily wellness checks, support and problem-solving to clients
  • Manage and distribute client supplies at all sites.
  • Assist with meal distribution.
  • Assist clients with daily living skills to include RV management and ADLs.
  • Coach and support clients as they transition from living on the streets to living indoors.
  • Ensure quality client assistance with intensive housing searches, housing-readiness activities, and housing move-in and placements
  • Develop individual housing case plans and provide ongoing housing search activities.
  • Assist participants with housing applications and understanding and abiding by lease agreements.
  • Participate in daily team meetings and submit a daily log of service activities to the Supervisory Case Manager.
  • Create and maintain paper files and electronic (HMIS) case files to record all documentation.
  • Assist the Supervisory Case Manager to analyze and organize client data to be used in reports to present to funders on a monthly and quarterly basis.
  • Work closely with other agency Case Managers and Housing Navigators to ensure consistency in philosophical and practice approaches to services.
  • In collaboration with the Supervisory Case Manager, monitor the local housing rental markets by keeping abreast of market rental rates, supply and demands of housing and other related conditions to ensure effective housing searches and placements.
  • Develop partnerships with property managers and landlords throughout Alameda County and assist in the maintenance of a database of landlords and available properties.
  • Develop and maintain relationships with all local Housing Authorities in Alameda County.
  • Provide information to Respite clients about affordable housing options, housing subsidy programs (including the PSH, NED, COACH and RRH subsidy programs) to participants.
  • Assist participants with review of lease agreements and move-ins.
  • Facilitate and support the Respite client with Medi-Cal, CalFresh and SSI applications.
  • Attend and participate in all assigned program and external meetings.
  • Successfully complete HMIS Privacy and Security and Software training and all other assigned staff trainings.
  • Maintain confidentiality regarding clients, personnel and other internal agency affairs.

Qualifications, Skills and Abilities

  • Bachelor’s Degree in Social Services or related field preferred.
  • Previous experience conducting effective housing searches, developing landlord relationships and successful housing placements.
  • Commitment to serving homeless individuals. Prior 3 years’ experience providing case management services to homeless and low-income individuals and individuals with mental health or substance abuse issues.
  • General knowledge of local community resources.
  • Proficiency with PC Computer systems and Microsoft Office (work, excel).
  • Ability to interact in a supportive and professional manner with staff and clients of diverse cultural and economic backgrounds.
  • Good written and oral communication skills.
  • Good conflict resolution skills.
  • Ability to meet deadlines and complete multiple tasks in a timely manner.
  • Capacity to work independently and as part of a team.
  • Ability to maintain professional conduct, attitude and appearance at all times.

Special Requirements

  • Must have a valid driver’s license and clean DMV record.
  • Must be able to obtain background check clearance.
  • Must obtain and maintain First Aid and CPR certification.
