The program provides intensive case management to youth 16-25 and their families living throughout San Francisco. Our staff provide short-term, community-based services to youth as they step down from crisis services such as a psychiatric hospitalization, to ensure they are linked successfully to long-term support services.  The program’s goal is to help clients maintain stable mental health, access the right services to achieve wellness, and prevent future crises.

The Clinical Case Manager (CCM) supports a caseload of 10-15 clients who are either: a) experiencing a mental health crisis; b) moving towards a mental health crisis: or c) in the preliminary stages of recovery and stabilization following a mental health crisis. The CCM evaluates needs of clients through phone screening and crisis assessment; provides linkages to emergency services and develops a comprehensive care plan in collaboration with each client and their family. The CCM provides ongoing linkage and navigation services to ensure that each young person is securely connected to needed services, including outpatient, residential, housing, and substance use treatment programs.  The CCM provides behavioral health counseling and psychiatric care for up to 90 days for most TAY clients and for up to 180 days to address the needs of TAY who are identified as being at extremely high risk for a crisis episode and who need additional stabilization services in order to prevent a mental health crisis.  The CCM may also assist with family problems; suicide; stress management; problems with self-esteem and assisting clients in applying for and obtaining other social services while providing a safe and supportive environment for agency’s clientele and adhering to laws regarding confidentiality and reporting requirements.  Activities may include individual and group therapy, crisis intervention, case management, client advocacy, prevention, and education. While the bulk of services will take place during regular weekday hours and on Saturday mornings, case management staff will maintain a highly flexible schedule that allows them to provide crisis intervention services during evening and weekend hours as needed.  The CCM will maintain accurate detailed observation and progress records for electronic billing data entry.

Minimum Qualifications:

  • Master’s degree in Social Work, Psychology or related fields of study required
  • Must have 2 years of experience in case management and mental health services with severely mentally ill, dually diagnosed TAY
  • Experience with Medi-Cal billing/documentation
  • Knowledge of the MHSA philosophy of care a plus
  • Willingness to work from a strength-based, harm reduction, client centered model with an emphasis on evidence-based practices

Clinical supervision is offered for ASW and AMFT.
