Job Information

The Intensive Case Manager provides intensive and comprehensive case management services to the diverse residents residing in the agency’s housing portfolio. This includes ongoing resident engagement, collaboration on housing retention, and providing support to low-income residents including formerly homeless families and single adults, individuals with various mental health and substance use histories. This position delivers services onsite and in community settings, ensuring implementation of services at all supported locations meet internal assigned standards and objectives.  Monday through Friday 9AM-5:30PM, plus some nights and weekends

Qualifications 

  • BA degree
  • Bilingual and biliterate in English and Spanish or Chinese
  • 1-3 years of experience providing intensive case management services which include working with vulnerable populations such as low-income individuals and families experiencing mental health and/or substance use issues, formerly homeless, low income and supportive housing,  veterans, TAY, immigrants, and seniors.
  • Experience and knowledge with providing crisis response, intervention and follow up (including but not limited to child abuse/ elder abuse reporting) implementing mandated reporting policies and procedures
