Social Worker ASW
We are recruiting to fill an ASW position within the innovative Street Medicine program. The ASW works as part of an integrated, multidisciplinary team including a community health worker, registered nurse, and primary care providers to assess, treat, and support the holistic needs of individuals living in encampments and on the streets experiencing homelessness. The ASW works with patients and partner organizations to provide short-term intensive transitional case management until a patient has stabilized enough to transition to a conventional system of care (primary care and a community mental health clinic or Intensive Case Management program) or has been placed in a higher level of care. As a critical member of this team, the ASW is responsible for outreach and engagement, mental health assessment and intakes, care plan development, intensive short-term case management of a 25-patient caseload, and collaboration with licensed mental health clinicians and the more extensive medical and mental health system. The ASW is also the team lead in housing assessments and supporting patients to secure housing when matched to a housing resource. The ASW will be a trusted member of the community they will work with. This relationship will enable the ASW to liaise between health, social services, and housing resource providers.

- CA driver’s license, as this position requires the employee to drive to and from encampments for outreach, and each team does 20 hours of outreach per week. Different team members can rotate who drives vehicles.
- Knowledge and experience providing mental health services to high-risk populations.
- Experience in clinical case management and harm reduction.
