Clinical Case Manager- Case Managers undertake a range of key responsibilities for clients that relate to healthcare, behavioral health, and housing. Case Managers will perform bio-psychosocial assessments and create care plans for clients and manage delivery of services with community partners. This position will share time between our Clinic and our Medical Care Coordination Department. Clinic and community based. Clinical supervision is offered.

ESSENTIAL DUTIES AND RESPONSIBILITIES

· Independently manage assigned caseload.

· Provide in-person, community-based case management services that are strengths-based, client-centered and trauma-informed.

· Complete comprehensive biopsychosocial assessment to evaluate needs and create appropriate care plan.

· In partnership with clients, develop mutually agreed upon care plan goals and steps to be taken toward meeting those goals.

· Responsible for overall care coordination and communication between multidisciplinary providers to ensure provision of services and implementation of care plan.

· Provide crisis intervention for immediate housing needs, medical, mental health and substance use issues.

· Work collaboratively with internal Community Supports providers.

· Make recommendations and referrals to governmental or community-based programs, benefits and services. Directly assist clients with linkage.

· Assist clients to access medical, mental health and substance use treatment providers, including accompaniment to appointments as needed.

· Teach activities of daily living and other skills necessary to maintain independence in the community.

· Act as an advocate for clients with social services, community-based agencies, health and behavioral health providers.

· Work with hospital staff on appropriate discharge plan and follow up post discharge to assess and modify care plan as needed.

· Provide health promotion and self-management training as appropriate.

· Function as a liaison between departments bridging communication gaps and coordinating between case managers and staff.

· Assist clients to function at most independent level possible by addressing ongoing psychosocial needs once clients are stabilized and not in crisis situations.

· Complete documentation, data, and billing requirements in a timely manner per published procedure and timelines.

· Attend and participate in all mandatory meetings and trainings including group, individual and triadic supervision.

· Maintain strict confidentiality of patient information and ensure that patient privacy is maintained.

Qualifications:
-ASW/ LCSW
-2 years experience with vulnerable populations
