Our Street Outreach programs work to engage individuals who are currently homeless and living on the streets in an effort to provide and link them with services that will ultimately lead them to stabilize in housing long-term. As part of the Outreach team, the Clinical Case Manager helps clients break the cycle of homelessness by providing individualized and comprehensive support throughout their transition from street to housing.

Responsibilities of the Clinical Case Manager include:

  • Performing persistent and frequent outreach in all areas covered by the contract where those experiencing homeless reside and congregate and responding to community requests for street outreach intervention
  • Coordinating intake and needs assessments for all clients and completing biopsychosocial assessments that drive the intervention and supportive services
  • Utilizing assessment results to collaborate with clients to develop individualized service plans (ISP) that address barriers to obtaining housing and services
  • Providing on-going case management support to support ISP goals including advocacy, assistance with procuring documentation and services such as identification or disability income, and transportation in the company vehicle to appointments
  • Providing behavioral health interventions designed to mitigate harmful impacts of mental health symptoms and/or functional barriers
  • Maintaining expertise of pertinent community services and resources to effectively provide information, referrals, linkages, and advocacy that assist clients in obtaining permanent housing
  • Identifying appropriate permanent housing options for clients based on their needs, providing assistance with housing location and application and advocating for clients with prospective landlords
  • Developing relationships within the community, including local businesses, landlords, and other service providers, to identify new and existing opportunities and build relationships to better assist clients in accessing resources, employment services and housing

QUALIFICATIONS
To be considered for this position, we require all candidates:

  • Hold a Master’s Degree in human services field
    • Strong preference for Social Work (MSW) or Marriage and Family Therapy (MFT) as the field of study
  • Have at least 1 year of experience working with vulnerable populations, particularly those with health, mental health, and substance abuse issues
