Care Manager/ Social Worker

This is temporary, part or full time- 30 – 37.5 hours/ week and the work must be done between 8AM and 5PM, M – F. Position is in person, starts 1/2/23 and the assignment has no end date. The MSSP Care Manager works as part of an interdisciplinary team to provide home assessment and case management services to clients of the Multipurpose Senior Services Program (MSSP).

Job duties:

• Provides in-depth assessment/reassessment services to functionally impaired seniors and their families, identifying functional limitations that limit independent living, in compliance with MSSP requirements and Agency standards for performance
• Determines the level of care management services appropriate for clients and provides services in compliance with community and Agency standards
• Develops, implements and monitors care plans for clients, implementing services and providing follow-up and monitoring as described in the care plan
• Identifies and develops support systems for clients, consulting with other members of the interdisciplinary care management team
• Provides reassessment services on a regularly scheduled basis
• Refers clients to special consultants to provide detailed evaluations and recommendations to improve functionality
• Provides consultation and services to caregivers
• Presents cases to a multidisciplinary team
• Represents agency at community meetings and interagency client conferences

Qualifications:

• BA degree in social work, psychology, counseling, rehabilitation, gerontology or sociology
• 2+ experience working with seniors or commensurate experience
• Proven possession of knowledge, skills, and experience necessary to perform core functions of case management
• COVID vax required
