|| TAY Case Manager
||$22 / hour
The Case Manager provides intensive, weekly case management for Transition Age Youth (TAY) living at single site affordable housing locations. Through comprehensive case management, the Case Manager helps youth to identify and attain goals in the areas of employment and education, self-sufficiency, health and wellness, relationships, professionalism and personal advocacy. The Case Manager also provides on-site, drop in resource and referral support for TAY residents as needed and coordinates TAY community building activities that promote independent living skills. This position works closely with Affordable Housing partners to ensure TAY housing stability.
BA Social Welfare, Psychology, or equivalent required
At least three years experience working with transition age or foster youth with complex presentations including presentations of chronic trauma.
Must be comfortable working in community settings.
High level of personal accountability for the quality and impact of work
Excellent communication skills, professional demeanor, sound judgment, and strong organizational skills
Experience working with youth who have mental health and substance abuse issues
Must be able to pass a Community Care Licensing background check
