This role will be temporary, full time, Monday to Friday 8:30 to 5:00 and should last about 3 months

The Services Coordinator helps to promote programs and advance service objectives at the assigned property under the direct supervision of the Program Manager. The Services Coordinator acts as the integral link between resources and referrals available through service providers for residents.
Job Description:
1. Provide General Service Management from intake, education on services available, assistance with application procedures, and referral of residents to service providers in the community. Services may include meals-on-wheels, transportation, home health aides, homemakers, financial assistance, counseling, preventative health screening, and other needed services.
2. Housing Success/Eviction Prevention: Orient new residents and assist with accessing Rent/Utility assistance, Case Management, Housekeeping services, and other connections to resources required to prevent notices and evictions when possible. Assist and monitor residents to ensure satisfactory tenancy.
3. Development of Resource Directory: This directory is tailored to the needs of the community and may include a listing of state and/or local service providers that residents can contact for assistance.
4. Implementation of a series of educational presentations (aka curriculums) around topics relating to health care, agency support, life skills, referral sources; etc.
5. Assist residents with building support networks with other residents, family members, and friends

6. Facilitate the formation of Resident Associations and/or the promotion of Resident Ambassadors within the property. These individuals will assist fellow community members in planning social events, organizing activities, and leading discussions around daily life matters.
7. Under supervision of Program Manager, maintain relationships with service providers by facilitating communication with community case managers, social workers and agency providers; support the delivery of direct supportive services when appropriate.
8. Act as team liaison with property management.
9. Network with community agencies and providers to seek out new service partnership opportunities.
10. Responsible for consistent documentation of different types of services provided to each resident as well as to groups of residents.

Qualifications:
• BA Social Work, Psychology, Gerontology, or Health Education preferred (Combination of education with equivalent work experience accepted)
• 1-2 -year of experience as a Service Coordinator or experience coordinating services for older adults/ seniors

• Satisfactorily pass required background and fingerprint checks
• Must have a valid California Driver’s License, auto insurance, and reliable transportation
