We are hiring for a LCSW to work for a start up healthcare company that uses patient centered approach and new modern technology to support renal patients. Our client is based in SF, it is a specialty kidney care provider of integrated nephrology and dialysis care for people with Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD). Our client delivers world-class, technology-enabled multidisciplinary care both in-person and virtually to achieve the best outcomes possible for patients and the best value for partners, keeping patients healthy and out of the hospital, accelerating access to transplant, and increasing home dialysis adoption. We are looking for the social worker who wants to lead innovation in kidney care! This is a hands-on, combined clinical and leadership role. The applicant must have outstanding clinical credentials and a demonstrated ability to thrive in a fast-moving and constantly evolving environment, and be equally comfortable and excited to develop company-wide strategy and roll-up their sleeves to deliver hands on patient care. The successful applicant will also have strong interest in process evaluation, clinical practice design and quality improvement.

Required Qualifications
● 5+ years experience in the delivery of care to patients with CKD and ESRD OR other chronic disease management.
● LCSW

Because of the nature of this job and the high level of responsibility only candidates with LCSW and at least 5 years of post masters experience in renal care or chronic diseases will be considered. We also need the candidate to have strong macro experience and leadership experience.

This company is a start up, founded 3 years ago in San Francisco, they are also serving patients in Oregon. They are looking for a CA LCSW whom is open to getting licensed in other states eventually. This person can do a lot of work remotely but must be able to travel to Oregon and SF often and also to new territories. At the beginning this person will work directly with patients and also work on a macro level developing the SW department and being a strategic leader providing insight on how to better provide innovative care to renal patients. Pay is $90k – $100k with the opportunity to make bonuses and have stock. This would be the companies first LCSW on staff.
