Job Information

We are in need of a FT temp who can provide intake and assessment to the program participants, and conduct data entry and analysis to help them secure Permanent Supportive Housing (PSH) or Rapid Rehousing (RRH). Other responsibilities include outreach, VI-SPDAT assessments, screening for eligibility, assisting clients to be document ready, data entry and analysis on multiple data systems, including the Homeless Management Information System (HMIS), assisting Program Manager in facilitation placement meeting, coordinating service plans with providers and participant in housing placement and case management committees. The coordinator will represent the agency in the community with other service agencies in the County.

QUALIFICATIONS
Knowledge of:
 Principles and techniques of engaging chronically homeless people;
 Confidentiality laws and regulations, mandated reporter protocols, familiarity with adult protective services;
 Principles and techniques of public social service programs; Federal, State, and local laws, rules, policies and regulations governing public assistance programs;
 Principles, procedures and techniques, trends and literature of the chronic homelessness;
 Principles of counseling, motivation casework and case management; interviewing, case management record keeping and problem-solving methodology;
 Proficiency on Excel/ Work/ PowerPoint/ HMIS; and
 Principles of Housing First and Harm Reduction.
2 year degree or 2 years of solid relevant experience required.
