Case Manager II

This is temp, FT for 3 – 6 months.  Our client serves clients 55+ with complex medical and psychosocial problems. Social Workers are in a key role providing biopsychosocial assessments, care coordination, and case management services for a client caseload in collaboration with a dynamic interdisciplinary team. The Social Worker supports client self-determination and promotes client-centered care while engaging in development of behavioral and other health interventions to enhance quality of life in a center-based, geriatric primary care setting.
