The Case Manager plays a critical role in supporting a client’s progress as they move through the stages of recovery, and assists them to transition from the behavioral health system. The Case Manager is responsible for establishing contact with newly referred clients and developing a collaborative relationship that will facilitate their participation in treatment. The Case Manager is responsible for maintaining consistent contact with consumers, deepening the treatment relationship, assisting consumers to identify life goals and linking them to community resources that can support their wellness and recovery. Successful candidates will have a working knowledge of motivational interviewing, harm reduction and abstinence-based models of treatment, candidate must have a MA or BA + 2 years experience, no degree is required if candidate has 4 years of relevant experience. This is a community based position so a car is required.
