Job Information

4 month temp assignment starting 3/1, FT, M – F. Provide case management and/or social service coordination for residents including crisis intervention and client advocacy; Maintain client documentation and records both in written and electronic form. Develop individual client plans, working with client, family, building staff, program and community resources to implement plan. Serve as liaison/advocate for residents to property management staff, outside social and health agencies; Provide current information and referral regarding services and community resources to residents and staff through case management, residents’ meetings and programs, staff meetings. BA required + 3 years of case management experience with homelessness, mental health and/ or domestic violence.
